The credit monitoring websites to be used are Credit Karma, Experian, and MyEquifax.

I confirm that I am the client and that all the information provided above, including my signature, is true and accurate to the best of my knowledge. I agree to hold Frazier Credit Services and/or its assignees harmless, and I release them from all liability for any loss or damages resulting from any misrepresentations made by me.

I understand and acknowledge that I am enrolling in a flat-fee credit repair program, either the 30-Day Expedited Program or the 90-Day Tier-Based Program. I further understand that all fees are nonrefundable.

I also acknowledge that Frazier Credit Services will only address the credit items identified at the start of the 30-Day or 90-Day Program. Any additional or newly reported items not included in the original scope of work will require a separate fee, should I choose to have Frazier Credit Services address those items.
